Sacramental certificates may only be issued once the sacrament is administered and the information is recorded in the appropriate register book. No certificates may be given at the sacramental celebration.
Below are sample sacramental certificates. In the Diocese of Sacramento, parishes may design their own sacramental certificates; however, all parish sacramental certificates must include the wording as shown on these samples. In addition, the pastor's original signature and the parish seal must be on all certificates. If the pastor is away, he may delegate this to the parochial vicar or deacon in his absence. Letters are not acceptable as a substitute for a sacramental certificate.
